Request a copy of your health record

You are legally entitled to request a copy of all or part of your health record. Requesting this information is known as a Subject Access Request (SAR).

Strict guidance is followed on data protection and confidentiality to ensure copies of a health record are provided to the correct individual. We need written consent to discuss any aspect of your health record with anyone other than you. Please bear with us whilst we make sure all the criteria are met before we release any records.

To request a copy of your health record

You can view your health record in the NHS App. If you don’t currently have access to your online services, please complete our dedicated form to request access here.

You can submit an online admin request via Anima between 8am – 6:30pm, Monday – Friday.

You can print our digital Subject Access Request Form. Once completed please return this to the practice by visiting any one of our three locations.

Alternatively, you can either call 01329 845777 or visit the practice during opening hours.

Please note, requests have a legal turnaround of 30 days, from the date they are submitted with the practice. In instances where your request is complex and requires further review, we will formally write to you to confirm this and request an extension.
