Some reports or forms require a physical examination, others are completed via a review of your medical record. Your documentation will make clear what is needed.

Some services provided by our Practice are not covered under our contract with the NHS. These private non-NHS services can attract charges. The doctors reserve the right to refuse to undertake such work if they do not feel it is appropriate and will always make NHS work their priority.
