Social Prescribing Day is an annual and national celebration of the people, organisations and communities who make social prescribing happen.

We celebrated on Thursday 14th March at our weekly patient drop-in at St Columba Church. Our Social Prescribers and Health Coaches hosted a fantastic celebration alongside their Solent Mind advisor colleagues.

For many people, problems like loneliness, bereavement and unemployment can have a huge effect on their overall health, but the solution may not require a prescription or a doctor. Our Wellbeing Team are here to listen and work with patients to help find the way forward. Social Prescribing connects people to non-medical support to improve their health and wellbeing.

Meon Health Practice GP Partner, Dr Rem Lee personally thanked the team for their hard work and commented “the Wellbeing Team have transformed the support we are able to provide patients who need non-medical assistance”.

The team were joined by many local support services including Citizen’s Advice, Community First Fareham, Andover Mind, Healthier You – NHS Diabetes Prevention Program HIOW (Xyla Health & Wellbeing) & Bee-Released Counselling. It was great to see many patients who our Wellbeing Team make a big difference to.
