You are legally entitled to request a copy of all or part of your health record.
Strict guidance is followed on data protection and confidentiality to ensure copies of a health record are provided to the correct individual. We need written consent to discuss any aspect of your health record with anyone other than you. Please bear with us whilst we make sure all the criteria are met before we release any records.
To request a copy of your health record
You can view your health record in the NHS App.
You can submit an online admin request via Anima between 8am – 8pm, Monday – Friday.
Alternatively, you can either call 01329 845777 or visit the surgery during opening hours.
