Why are we moving online?
Moving long-term condition management online, we hope to improve your overall experience with our practice by:
Reduce the time you have to wait for an appointment
Provide you with easier access to clinicians when you need it
Give you a clear management plan much quicker
What will you have access to?
You can access Suvera’s patient secure patient portal as part of our online clinic. From here, you can submit information about your health from home.
Getting your invite
You’ll receive a text message inviting you to use the online clinic and a link to the patient portal.
To access the patient portal, log in using your full name and date of birth.
Submitting information through the patient portal
You’ll be able to answer the health questionnaires through the patient portal and submit data like blood pressure if we need it.
When you share information about your health, a dedicated and specialised care team will review your data. They then provide clinical support all year round for your condition management.
Any information you share is safe and secure and won’t be shared with anyone but your GP practice.
Ongoing appointments
A member of the care team will review the information you share and decide on the appropriate actions or next steps.
Most of your appointments for long-term conditions will be done via phone or video call. In this appointment, you will discuss your long-term conditions, medicines, and other important lifestyle factors like diet and exercise.
ChatHealth is a confidential text support service for parents, carers, families and young people in Hampshire. Trained local nurses will be on the other side responding to your messages and are experienced in a wide range of health and wellbeing concerns you may have. As well as giving advice, the team can signpost to appropriate services. Any text sent outside opening hours will receive an automated message with advice of where to get help if their question is urgent. Out-of-hours texts will be replied to within one working day.
The service is provided to the whole of Hampshire by the Hampshire and IOW trust. They can deal with almost anything that’s not to ‘medical’ like potty training help for young ones, concerns about speech, safe sex advice for older teens and much more.
The Spring COVID-19 vaccination campaign has now concluded nationally, ending on 17th June 2025.
Details regarding the Autumn 2025 campaign will be shared as soon as we receive confirmation on the eligible cohorts, campaign dates, and vaccines to be used.

The Spring COVID Vaccination Programme will run between 1 April & 17 June.
The following patients are eligible for a Spring COVID Booster:
Individuals aged 75+ – including anyone turning 75 by the 17 June 25
Anyone living in an older adult care home (over 65) regardless of age.
Immunosuppressed individuals aged 6 months+
We will contact all eligible patients to arrange a vaccination appointment including those who are housebound.

We recognise that many people in the LGBTQ+ community are often reluctant to ask for medical help because of negative experiences they may have had in the past. At Meon Health Practice we support everyone regardless of gender identity or sexual orientation and strive to ensure all our patients feel supported and listened to.
Sexual Health
Let’s Talk About It is a service provided by Hampshire and Isle of Wight Healthcare NHS Foundation Trust. They offer Sexual and Reproductive Health services across Hampshire, Isle of Wight, Portsmouth and Southampton including contraception, sexually transmitted infections (STI) testing, advice about pregnancy, HIV, sexual assault and much more.
If you prefer, we can add a “known as” name to your record so we as a practice can call you by your preferred name.
Changing Your Title
You can change your title from Mr to Miss/Mrs/Ms or from Miss/Mrs/Ms to Mr without changing your gender.
Tell Us Your Pronouns
Pronouns replace a person’s name when referring to them. While she/her and he/him are the most common third person pronouns, some people use gender-neutral pronouns such as they/them, ze/hir/hirs and xe/xem/xyr. If you want to change your pronouns, just let us know.
Gender Change on Medical Records
Patients may request to change gender on their patient record at any time and do not need to have undergone any form of gender reassignment treatment to do so.
We must advise you, the patient, that changing your gender on your records would mean that you will be supplied with a new NHS number which is not reversible, and we will need to register you as a new patient. Your old NHS number will no longer exist and your existing medical information will be transferred into your new medical record minus any information relating to your previous identity.
To revert to your previous gender, we would need to issue you with a third NHS number and transfer your then existing medical information minus any information relating to your previous identity to your new record.
We are not allowed to change NHS numbers and gender markers for children and young people under 18.
Information for Transgender and Non-Binary People – NHS Screening Programmes
Upon changing your gender, it’s important that you understand that you will not receive automatic invites for screening for certain cancers and conditions and that you read and understand this leaflet NHS Screening Advice for Trans People so that you are fully informed.
An NHS Shared Care Agreement is a formal arrangement between you, our practice and an NHS specialist clinic. Under this agreement, our practice will prescribe specialist medication and undertake appropriate monitoring, while the specialist clinic will continue to review your care at regular intervals. This arrangement ensures that responsibilities for prescribing are clearly defined and governed by local policies, maintaining patient safety and high standards of care.
Shared Care Agreements – Private
In recent years, more patients have sought private treatment and asked GPs to prescribe specialist medications recommended by private specialists, this differs significantly from NHS Shared Care Agreements. In line with other GP practices, we do not participate in Shared Care Agreements with private providers or issue new prescriptions for specialist medications initiated by private providers, patients must obtain these prescriptions directly from their private specialist. Shared Care Agreements with private specialists is only possible when this service is being provided for the patient on behalf of the NHS.
Memory Clinics (dementia reviews)
We offer memory clinics to support patients who have received a dementia diagnosis. These dedicated clinics are usually held at our Highlands surgery on Saturdays which tend to be less busy.
The clinic provides an opportunity to discuss your health with a healthcare professional in a calm and safe space. You are welcome to attend with a family member or carer if you think this would be helpful.
We will try to offer an alternative appointment if you are unable to attend on a Saturday or at our Highlands surgery.
We will contact you each year to invite you for your review. Of course, if you you have health issues in between reviews, do contact us.
Our Wellbeing Team offer a variety of patient-drop in sessions. These are an opportunity to come along and chat to our Social Prescribers and Health Coaches about any non-medical issues.
The team also offer sessions for patients and carers who need support with cancer and pain management.
Quarterly (2025 dates will be available soon) 7:15pm – 8:15pm at Whiteley Surgery
Our Wellbeing Teamhost this group for patients who are experiencing chronic pain e.g.: fibromyalgia, joint pain, inflammation, sports injuries, etc. The group often includes guest speakers on a range of topics and therapies and is an opportunity for patients to share experiences.

Our Wellbeing Team offer three gentle low-intensity walks each week. These are led by one of our health coaches. These walks take place, usually whatever the weather!

SurgeryPods are available in all our surgery reception areas
A SurgeryPod is a machine connected to a computer which can accurately take readings such as weight and blood pressure measurements. You can use a SurgeryPod at your own convenience during our opening hours, without making an appointment.
